As a patient of Commonwealth Orthopaedic Centers are responsible for all fees which you incur as a result of being our patient. We accept insurance assignment and will file your insurance claim(s) for you. You are responsible for all co-payments and/or balances as required by your insurance plan. You must bring your insurance card(s) with you for all visits to Commonwealth Orthopaedic Centers, if you do not your appointment will be rescheduled. You must obtain all referrals required by your insurance company from your primary care physician prior to your visit to us. (We do not secure referrals for our patients.) All co-payments and co-insurance are due at the time of your visit. A patient who is younger than eighteen (18) must be accompanied by an adult who is responsible for his/her co-payments, co-insurance, and/or deductibles.

Pre-certification by your insurance company is required in advance of all elective surgeries scheduled with us. Our staff will contact your insurance carrier for required approvals. All co-insurance and/or deductibles must be paid in advance of your surgery.
