Your First Visit

Patient Registration Forms

If you are a new patient, please arrive 20 minutes prior to your appointment to complete all paperwork or fill out and print the forms online and bring with you to your initial visit.

Please bring the following medical records:

All x-rays and films of MRIs and Bone Scans
All test reports and medical records
Insurance card and all co-pays
Please be sure to have proper referral documentation

If you have any questions please contact the appropriate Doctor's office.

If you have any Billing questions, please call (513) 557-4270 and our Billing Representative will be glad to assist you.
