If you are a new patient, please arrive 20 minutes prior to your appointment to complete all paperwork or fill out and print the forms online and bring with you to your initial visit.

Please bring the following medical records:

• All x-rays and films of MRIs and Bone Scans
• All test reports and medical records
• Insurance card and all co-pays
• Please be sure to have proper referral documentation
